Supports that we provide
Group / Centre Activities
Assisting people in participating in their preferred activities.
- Community, social, and recreational activities centred on groups.
- Grocery shopping, cooking, personal care, gardening, and other domestic duties are examples of life skills development.
- Improved health and wellness – for example, health and wellbeing education and awareness, group exercise activities, walks, swimming, and yoga.
Innovative Community Participation
Supporting and growing participants’ ability to participate actively in communal, social, and civic events. This encompasses both supporting and improving participants’ abilities to participate in these activities.
Assisting and/or overseeing personal daily duties in order for the participant to live as independently as feasible. Individualized services can be delivered in a variety of settings, including but not limited to the participant’s home.
Household Tasks
We provide expert services to all of our clients at Unified Health and Care. Our disability care services consider all of your individual requirements and expectations and provide personalised assistance.
If it’s part of your National Disability Insurance Scheme plan, we can help with cleaning, such as:
- Changing the sheets
- Bath or shower cleaning
- Putting the laundry out to dry
- Vacuuming
- Cleaning the carpets
- Meal preparation, shopping, and other details are discussed before the service begins.
Assist-Travel / Transport
Personal transportation aids and equipment, as well as training to use public transportation, are examples of transportation assistance.
For participants who are unable to travel alone, transportation subsidies may also include the reasonable and required costs of taxis or other private transportation options.
Transportation supports are exclusively for participants; they do not cover travel for family, carers, or support providers.
Participate Community
Individuals with NDIS funding can participate in a variety of social activities in their community and beyond. Peer support and community participation are essential for social inclusion. As they interact socially, participants gain confidence and independence.
The following are just a few examples of social activities:
- Wednesday is bowling night.
- Bus tours and expeditions throughout the region for swimming
Daily Tasks / Shared Living
For participants funded by NDIS for Daily Tasks/Shared living we offer:
- Assistance with daily personal activities
- Assistance with household tasks
- Assistance in Shared Living Arrangements, and
- Supported Independent Living (SIL), Short Term Accommodation and Assistance (or respite).
Assist-Personal Activities and Development-Life Skills
Unified Health and Care assists individuals in learning daily living skills through the NDIS. By
Assisting with personal activities while also learning new skills.
To do this, Unified Health and Care offers a variety of supported possibilities in a variety of settings. These kinds of activities help individuals develop independence and social skills.
These include, but are not limited to, the following:
- Cooking \Cleaning
- Bills to be paid
- Budgeting
- Being self-sufficient
- Personal cleanliness is really important.

We provide ongoing support services to help you live the life you want.
